participate in voting, promotions, surveys conducted by LLC "Leopolis Jazz" (hereinafter - the "Organizer") on the web site www.alfajazzfest.com,

any other way provides or makes available to the Organizer your personal data

(hereinafter - the "data subject"), since:

fill in a form for ordering tickets online at web site www.leopolisjazz.com,

taking part in the voting, promotions, surveys conducted by the organizer of the site leopolisjazz.com,

provided by the organizers in any way your personal data, or any action which resulted in the Organizer can gain access to personal data of the data subject,

voluntarily provide to the Organizer unlimited, unconditional and irrevocable consent to the processing (including the gathering, recording, accumulation, storage, adaptation, modification, distribution, use, processing, depersonalization) of their personal data, which have become known to the Organizer in connection with a subject of personal data of any mentioned before.

2. Provided  in paragraph 1 hereof consent to the processing of personal data provided by the organizers the right to process personal data for the purpose of committing any action organizer that Organizer must take to meet their obligations to the subject of personal data , to exercise their rights, and / or any other purpose of the processing of personal data which is determined by the Organizer.

3. Personal data will be processed / stored during the period of no more than is necessary in accordance with the above-mentioned purposes of processing personal data.

4. Amount of personal data, which will be carried out with respect to the processing of personal data and which may be included in the database of personal data to the Organizer, defined as any information on the subject of personal data, which has become known to the Organizer.

5. Performing any of the acts stipulated in paragraph 1 hereof, the personal data subject to the Organizer provides its consent to change these Terms defined purpose of processing personal data by posting a message on the official website www.leopolisjazz.com and / or by sending SMS to the provided by the subject personal data to the Organizer phone number of the subject of personal data.

6. Consent to the processing of personal data, which is the subject of personal data provided by the commission of any of the actions provided for in paragraph 1 hereof, the commission does not require notification of the transfer of personal data subject's personal data to third parties in accordance with Article 21 of the Law of Ukraine "On Protection of Personal data".

7.
